Situation:

At least 21 people were killed and five others injured over the weekend when a bus plunged into a ravine in northwestern Colombia, officials said. The accident occurred Sunday in a rural area about 78 kilometers (48 miles) east of Medellin, the capital of the northwestern province of Antioquia, and near the town of San Luis. The bus, which belonged to a private transportation company, had started its trip in Puerto Nare, the town’s mayor, Jose Maximino Castaño, said. The bus, which was heading for Medellin, went off the highway at a place called La Josefina, Castaño told RCN radio. “The tally we have is 21 dead and five injured,” Castaño said, without providing details on the cause of the accident. The accident was caused by a combination of human error and mechanical failure, highway patrol director Gen. Rodolfo Palomino told reporters in Bogota.
